Does this plugin send unique coupon codes?

Yes, the coupon code is unique and will be generated dynamically every time (the code is generated by username and a combination of year, month, day, hours, minutes, seconds of when it was generated).
Customers will receive an email with a coupon code, every time they leave a comment. And users are identified by email address. Obviously multiple reviews on the same product will be ignored.

Will this plugin automatically send an email to the customer soon after inserting the review?

It depends on what you prefer. You can choose to send emails:

soon after posting a review
after approving the review
after reaching a certain number of reviews

Can you limit the discounts to one per customer? And only one review request per customer?

Yes, if the customer reviews the item multiple times, he/she will get only one discount. About your second question for one review request per customer, please, consider that this is not managed by this plugin, which does not send any review reminders to customers. It can be managed through another plugin, YITH WooCommerce Review Reminder, and yes customers will receive only one review reminder, even if they have placed more than one order.

Does this plugin support reviews for Facebook, Trustpilot eg. or just product reviews in the shop?

The plugin supports only native WooCommerce reviews and the ones submitted with YITH WooCommerce Advanced Reviews.

How to integrate MailChimp?

The plugin supports Mandrill which is a paid extension of MailChimp.

Can I set the coupon to be only valid if the user's cart is above a $ threshold? e.g. 'Review and get $5 off your next purchase over $50'?

Yes, the coupon settings have the option to configure a minimum amount to spend to use the coupon. You can also set other kinds of coupons as you can see in the official documentation.

Can I restrict the number of coupons per user/email address per product?

Yes, once the user added the review for a product and received the coupon, he/she won't receive other coupons if reviewing the same product again.

Do I need YITH WooCommerce Review Reminder plugin installed too?

It depends on what you really need. In order to make YITH Review for Discounts plugin work, you don't have to install Review Reminder too, but the two plugins work well together. Review Reminder lets you remind your customers of reviewing the products they have purchased; Review for Discounts lets you give coupons to your customers as soon as they review the products they have purchased. If you want to combine both features, you have to use them both, but if you need just one, you don't have!

Do I need to use Mandrill to manage and send emails?

No, you don't have to use Mandrill unless you need to send mass emails or manage email campaigns! YITH WooCommerce Review for Discounts works also without Mandrill!
